Toyota Avensis noise when in higher gears

my car has recently started making a horrible whirring/scrapping type noise when in mainly 3rd 4th and 5th gear upon acceleratiion although you can hear it a little in second we have had the passenger side shaft replaced and a second hand gear box fitted it has cost a fortune !!!!!!!!!! and the noise is still there :-( ...... any ideas ????

It could be: input shaft bearing worn, damaged main drive gear bearing, insufficient or poor condition transmission oil (drain and fill with proper grade oil. Check oil for water and/or debris),variation in engine torque (change the idle speed and see if noise goes away), worn, damaged or chipped gear teeth and/or synchronizer.
