Ford Focus hesitation in acceleration

Recently noticed a flat spot as I accererate hard.
Seems to come in at c.3,250 revs and goes away at c.3,750 when full power kicks in again - there is no mechanical noise, nor smoke, nor any other problem - just a lack of power. I've noticed it in first, second and third gears. It is like the turbo stops working between these rev bands - it still accelerates, but very much slower.
She has now done 111,000 miles (150 miles a day round trip to work), mostly motorway. I do drive quite steadily, rarely exceeding 2,000 - 2,500 revs most days (c.58-60 mpg). She is regularly serviced at a Ford garage and is due another at 112,500 miles.
Anyone any ideas?

Mass air flow sensors are quite common on these, unplug it and drive it, if it is any better then theres your fault

tomorrow when you go to work take the revs up to 5500 6000 revs and change gear the reason i have said this is because alot of cars that get driven steady tend to have more running problems than a car thats been driven a little harder but you will only have to do this once and then you should be ok for a few more months until the problem returns and then just repeat as mentioned

If I rev to 5,500 it would suffer permanent damage - it's a diesel so it maxes out at 4,250. Over the last week (c.1,000 miles) I've hit max revs in first, second and third many times to 'clear it out'. No difference at all. I have noticed though, on less than full throttle and on a steadily increasing throttle, it'll pull cleanly throught the flat spot.
